Sometimes (I'm not sure why) my pen tool reverts to a simple "+" (like the marquee tool). And Sometimes I can get it back and sometimes I can't. Any help? It's really annoying.

My first thought is to check your caps lock button ... that changes the look of your 'cursor' with many of the tools in photoshop.
