Photoshop CS4 - Interface Layout keeps changing to Default?

I am running Photoshop CS4 and I am having problem setting up the interface. The default set up when installed is a single column toolbar and the palettes on the right of the workspace are locked. Pic 1

I prefer the older toolbar smaller with two columns and the palettes unlocked with just the layer and history palette visible. Pic 2

I managed to get the interface set up the way I wanted, but the problem that I have is when I exit Photoshop and reopen Photoshop it reverts back to the default layout like (Pic 1).

Is there a way to stop Photoshop reverting back to the default layout?

Set up a new workspace the way you want, name it and save it. Window-Workspace-New Workspace

I managed to resolve the problem with a revert back to the default settings using Ctrl+Alt+Shift. There must have been an error with some of the settings files.
