Hey guys. Could really use some help here. Take a look at the attached image. I'm trying to make something similar, but with my own words and on a different angle. How can I use Photoshop to achieve this effect?

Type the text you would like and apply a drop shadow layer style. Rasterize the text, then use transform-perspective, or other transform tools to get the effect you want. Add a layer mask with a black to white gradient for the fade effect.

...and just use a mask to fade the top....
